يرتبط العمل عمومًا بحالة نفسية نادرة أحيانًا ما يُطلق عليها بطريق الخطأ «انفصام الشخصية»؛ حيث يوجد بداخل الشخص الواحد أكثر من شخصية مختلفة. وفي الحالة التي تطرحها الرواية، توجد بداخل الدكتور جيكل شخصيتان مختلفتان تمام الاختلاف من الناحية الأخلاقية، إحداهما طيبة في الظاهر، والأخرى شريرة. كان للرواية تأثير قوي؛ حتى إن عبارة «جيكل وهايد» أصبحت دارجة لتعني الشخص الذي يختلف توجُّهه الأخلاقي اختلافًا جذريًّا من موقف لآخر
